Our Hospice at Home team is here to support patients in the comfort of their own homes or in Care Homes, helping fulfill their wishes to stay in familiar surroundings. We understand that this can be a challenging time, so our team is dedicated to providing the right level of care and support for both patients and their families.

By offering expert assessment and symptom management at home, we ensure that patients receive the personalised, compassionate care they need—wherever they feel most at ease.

When you’re caring for someone, or living with a life-limiting illness yourself, worries don’t keep office hours. That’s why our advice line is here 24 hours a day, 7 days a week.

Whether you need urgent advice on symptoms or medication, reassurance in the middle of the night, or simply aren’t sure who to turn to — our experienced team is only a phone call away. You don’t need to be known to the hospice to get in touch.
